Gynecologic Benign Conditions
There are five main types of non-cancerous gynecological conditions that affect women’s reproductive organs which we treat at the Gynecologic Oncology Institute: advanced endometriosis, complex ovarian cysts, complex uterine fibroids/myomas, and pelvic masses, using minimally invasive surgery. We also perform complex reoperative gynecologic surgery and pelvic reconstruction for a variety of more complicated pelvic conditions. Overall, the treatments include a number of specialized techniques and surgical procedures. We offer the most advanced XiRA robotically assisted surgery, even for the most complicated of cases.
We DO NOT focus on basic gynecology as there are many great gynecologists in the community. We DO focus on complex problems, including cancer and those conditions noted above, that perhaps very few physicians and surgeons can tackle.
The benefits of excisional surgery for endometriosis are clear. This is a far better approach than tissue damaging burning or fulguration which does not reliably destroy the endometriosis implants and puts nearby organs at risk.
Endometriosis resection is the new gold standard for surgical management of endometriosis. Implants are identified and resected or removed with surgical precision. This is especially helpful to reliably cut out endometriosis in sensitive areas close to other organs, like the bowel, bladder or ureters.
Surgical excision or resection is possible using small incisions, the number of which depends on the amount of endometriosis that needs to be removed. Laparoscopy is often used for basic excision and may be sufficient for that purpose. The problem is that laparoscopy can be “clunky” due to the nature of straight instruments with limited mobility and a two-dimensional view from the camera. In the very best surgeon’s hands, it may become too imprecise for advanced resection. The amount of endometriosis that will be found during surgery and the exact locations are hard to predict.
Robotically assisted laparoscopy is the next-generation of precision endometriosis excisional surgery. Endometriosis resection using robotics offers a magnified 3D view (more precise) and instruments that have tiny wrists at the tips which makes even extremely difficult endometriosis excisional surgery possible. The key is not just the robotic machine technology but the surgeon’s expertise.
